If inflammation occurred after the initial surgery, it is advisable to proceed with revision surgery after the inflammation has completely resolved. In some cases, depending on the surgical outcome, revision surgery may not be possible until more than one year has passed. Revision Surgery Methods by Type

❶ When double eyelids always appear swollen

By 3 to 6 months after surgery, swelling should have mostly subsided and the shape should look natural, but in many cases the eyes remain swollen as if surgery had been performed yesterday. This can occur when fat or muscle below the double-eyelid crease has not been adequately removed. A natural eyelid line can be created by directly excising thick tissue or lowering the double-eyelid crease height.

❷ When both eyes are asymmetric

No part of the human body can be perfectly symmetrical. The same applies to the eyes, but if there is a clearly visible difference between the eyelid lines after eyelid surgery, revision surgery may be necessary. If one side is thicker or thinner, the other side is adjusted to make the lines match in thickness.

This may occur after a non-incisional suture technique. It can happen when fixation was too weak during the initial surgery. Revision surgery is performed to create a thicker double eyelid, either by removing a small amount of skin above the crease or by fixing the eyelid-opening muscle and skin at a point slightly higher than the existing double-eyelid line. In most cases, immediate revision surgery within one month is recommended.

If too much fat between the eyelid-opening muscle and the eyelid is removed, the muscle and eyelid skin may adhere at multiple points, creating multiple double-eyelid folds. Revision surgery is performed by releasing the muscles adhered to the eyelid and appropriately adding fat.

❺ When the eyes appear sunken after surgery

In some cases, a lack of upper eyelid fat can make the eyes appear sunken. In such cases, surgery may be performed to bring down and reposition orbital fat located deep within the eye area, or fat grafting may be performed.

❻ When scarring is severe

Scarring may occur in patients prone to keloids, in which healing scars become raised and uneven, or when stitches are removed too late. When only the scar itself is severe, scar tissue is directly excised. When the eyelid line is also involved, the scar is removed while making the double-eyelid crease shallower.
