# When the double-eyelid crease is severely thick

If the crease was designed too high during the initial surgery, making the eyes look like sausages, an incision is made including the original incision line. The overly high fixation is released, and the crease is lowered and re-fixed. If there is a possibility that a crease may recur at the original high position, additional procedures using fat grafting or surrounding tissues may be needed.

# When the double-eyelid crease is too low

If the double-eyelid crease was set too low during the initial surgery, it can be redesigned to the desired crease and improved by selecting a surgical method appropriate for the eye condition and design. If reduced eyelid elasticity causes skin to descend and cover the crease, the descended skin can be removed to create a double-eyelid crease at the desired height.

# When the double-eyelid crease has loosened

When the double-eyelid crease is weakly fixed, it may loosen over time. The loosened crease is redesigned, and an appropriate method suited to the eye condition—such as the natural adhesion method or an incisional method—is selected to minimize the chance of loosening again. However, if a more secure result is desired due to concern about recurrence, this can be addressed with an incisional procedure.

# When double-eyelid scarring is severe

This method is performed when scarring at the surgical site is severe. If sufficient eyelid tissue or skin remains, the scarred area is removed and the double-eyelid crease is corrected while preserving the existing tissue. If there is insufficient eyelid tissue after removing scar tissue and creating the crease, fat grafting to the eyelid can be used.

# When there are multiple double-eyelid creases

Multiple double-eyelid creases may occur when a previously created crease loosens or when unnecessary adhesions were formed during prior surgery. If no change in crease size is desired, an incision is made while removing scars, followed by dissection toward the upper crease to completely release adhesions; additional measures are needed to prevent re-adhesion. If multiple creases result from weak eyelid-opening strength, ptosis correction may also be required.

This may occur when the initial surgery was not performed according to the design or when the double-eyelid creases were fixed differently on each side. This type begins so that the double-eyelid crease is barely visible at the inner corner and gradually becomes wider toward the outer corner.

This is the most commonly seen double-eyelid crease among East Asians. It is suitable when the epicanthal fold is not severe.
