What Is Columella Surgery · Reconstruction?

The columella is a pillar-like structure connecting the philtrum and Nasal Tip, supporting the Nasal Tip, and is an important anatomical area.

The degree of projection, length, and shape of the columella can affect the perceived length and appearance of the nose, as well as the proportions and balance of the entire face. A severely retracted, deviated, or collapsed columella requires structural reconstruction rather than simple correction.

JT accurately analyzes various causes, including columellar depression, deviation, and asymmetry, and provides customized reconstruction with consideration for harmony with the entire face.

Ideal Columella Shape

✓ From the side, the columella is naturally exposed approximately 2–3 mm beyond the nostril rim,

✓ From the front, the columella is positioned straight in the center,

✓ And ideally forms an angle of approximately 10–15° with the nostril wings.

However, as the shape and degree of columellar exposure should be in harmony with the individual's facial shape and the nose's shape and length, rather than following uniform standards, the design should be determined through thorough consultation with the surgeon.

Cases Requiring Columella Surgery · Reconstruction

❶ When the Columella Is Retracted

The area beneath the Nasal Tip appears sunken

Less columellar show than the nostril wings, or barely visible

A short Nasal Tip with a retracted columella creates an upturned appearance

A long Nasal Tip with a retracted columella creates a Long Nose appearance

JT Plan: Depending on the degree of retraction, a septal extension graft is performed to reinforce the columella, and the medial crura of the Nasal Tip cartilage are secured to lower the columella. The position of the Nasal Tip is also corrected at this time.

❷ When the Columella Is Drooping

The columella appears excessively lowered from the side

Excessive nostril show

The nose appears long and the Nasal Tip appears heavy

JT Plan: The descended cartilage is set back or repositioned, and if necessary, a portion of excessively developed cartilage is removed to elevate the columella.

❸ When the Columella Is Deviated

The columella is shifted to one side when viewed from the front

The nose appears crooked

JT Plan: Depending on the cause, the cartilage of the columellar support is repositioned or reconstructed, and the alar cartilages are symmetrized to correct the central axis.

❹ When the Columella Is Retracted Due to Capsular contracture

The Nasal Tip becomes shorter and upturned

A sensation of the columella being pulled inward

The skin is firm with restricted movement

JT Plan: After removing scar tissue caused by Capsular contracture, the Nasal Tip support structure is reconstructed with Autologous Rib Cartilage to restore the position of the columella and Nasal Tip. The Nasal Tip is corrected to its normal position through Nasal Tip cartilage separation and skin Dissection.

Actual Cases of JT Columella Surgery · Reconstruction

Case 01 Doctor's Note

From the front, the columella was short relative to the nostril wings and the Nasal Tip projection was low, making the nose appear wide from side to side. After sufficiently considering the available range of Nasal Tip skin, the septum was extended and the support structure reinforced using Autologous Rib Cartilage. During this process, the columella was corrected downward to straighten the central axis. A Silicone Implant was used for the Nasal Bridge to create a natural connection with the elevated Nasal Tip and Forehead.

Case 02 Doctor's Note

This patient had a history of rhinoplasty prior to visiting us and sought Revision surgery due to a low Nasal Tip and dissatisfaction with the overall shape. The reason the mouth appeared more protruded in profile was the low Nasal Tip, retracted columella, and narrow nasolabial angle. A Nasal Tip support pillar was created with Autologous Rib Cartilage, the columella was corrected downward, and the nasolabial angle was improved to naturally restore balance and appearance in profile.

Case 03 Doctor's Note

The patient had previously undergone rhinoplasty, but the columella had drooped and an excessively high Implant made the nose appear long and large. In the Revision surgery, the Nasal Tip support was firmly reconstructed with Autologous Rib Cartilage, and the drooping columella was corrected. The Nasal Bridge Implant was also replaced with a lower-profile Implant to create a natural connection with the Nasal Tip.

Case 04 Doctor's Note

The patient had previously undergone rhinoplasty with Septal Cartilage and Ear Cartilage, but over time the Nasal Tip and columella drooped, appearing excessively lowered from the front and making the nose look long. As the patient was reluctant to use Autologous Rib Cartilage, Donor Rib Cartilage was used to extend the Septal Cartilage and reconstruct the Nasal Tip support. The drooping columella was also appropriately corrected to improve the elongated appearance from the front.

Case 05 Doctor's Note

The patient underwent rhinoplasty with Septal Cartilage 20 years ago, but over time the columella became deviated and asymmetrical, and the patient sought revision correction. Using Autologous Rib Cartilage, the Nasal Tip and columellar support structures were firmly reconstructed in alignment with the center, then naturally refined with Ear Cartilage. The columella was naturally lowered for correction, creating a softer profile line as well.

Case 06 Doctor's Note

The patient had previously undergone rhinoplasty using Ear Cartilage and presented with a short, upturned Nasal Tip accompanied by Nostril Asymmetry Correction. To stably lengthen the short nose, stronger Autologous Rib Cartilage was used to reinforce the Nasal Tip support structure. The Nasal Tip point and columella were lowered together to improve the upturned appearance and naturally balance the frontal and profile views.

Biological Tissues for Rhinoplasty

Septal Cartilage

The Septal Cartilage inside the nose has appropriate thickness and strength, making it suitable as a pillar for supporting the Nasal Tip.

Ear Cartilage

Cartilage from inside the ear is used to shape the Nasal Bridge/Nasal Tip area.

Autologous Rib Cartilage

Cartilage is harvested from the end of the rib. Because it is sturdy, it can support the nose with greater strength.

Donor Rib Cartilage

This is Rib Cartilage from another person that has been processed to prevent immune reactions. As a separate harvesting procedure is not needed, it can be used conveniently.

Autologous Dermis

This is autologous dermis harvested from the groin or tailbone area, used when the skin of the nasal dorsum is thin or the Soft Tissue of the nose is insufficient.

Autologous Fascia

This refers to a thin membrane surrounding muscle; the temporalis fascia located mainly in the scalp above the ear or temple area is used.

Nasal Bridge Implants

Silicone implant

The most commonly used material, it is the safest material approved by the U.S. FDA.

AlloDerm

A Dermal Graft for transplantation made by removing cellular components that may trigger immune reactions from another person's dermal layer, leaving only the tissue structure composed of collagen and elastic fibers.
