The procedure to lower an existing Double Eyelid Surgery crease that is too large or high is commonly called double crease excision. A new incision is made below the existing double-eyelid incision, and the skin between the original and new incision lines is removed to create a new double-eyelid crease.

Double crease excision is a highly advanced procedure that requires intense concentration. Good results can be achieved only by minimizing tissue damage.

In double crease excision, there is often insufficient tissue in this area, which can lead to multiple creases, a triple line, or hollowing of the upper eyelid. This makes the procedure difficult, as the tissue must be elevated with great precision.

When there is insufficient skin and Soft Tissue, making it difficult to remove the existing scar, it is challenging to remove both the skin and scar to create a new double eyelid. In such cases, the existing scar must be released through a new crease line. This requires highly advanced and delicate surgical technique. Therefore, the procedure should be performed only when there is extensive experience and the ability to predict natural-looking results.

During surgery, excessive bleeding must be avoided, as it may cause severe scarring. An ergonomic connection between the skin tissue and the eyelid-opening muscle (levator palpebrae superioris) is required, and meticulous skin suturing should ensure that the scar is not easily visible even when the eyes are closed.
