Causes of Revision Rhinoplasty and Treatment Methods

# When the nasal tip becomes lower

If the nasal tip lacked support because the tip cartilage was weak and the columella was short, the cartilage may gradually droop over time. This can be corrected by extending the nasal septum itself or reinforcing the columella to elevate the nasal tip.

# When the nose has shortened due to contracture

This refers to a condition in which the nose becomes upturned and firm, causing the nasal tip to shorten. It is corrected by removing the implant and capsule, stretching the skin and nasal cartilage, and then inserting appropriate autologous material or an implant according to the condition.

# When the nasal tip shape is blunt

This may occur when the cartilage was not properly folded or treatment of subcutaneous fat was insufficient, resulting in a blunt nasal tip cartilage shape. It is corrected by repositioning the cartilage or grafting nasal tip cartilage in a more appropriate shape and size.

# When the nasal bridge feels too high or too low

This may occur when the height of the implant inserted in the nasal bridge was inappropriate. It is corrected by reshaping and reinserting an appropriate implant while considering overall facial harmony together with the nasal tip.

# When the nasal bridge feels too wide or too narrow

This refers to when the line extending smoothly downward from the starting point of the eyebrows and the brow bone, along the left and right width of the nasal dorsum, is too wide or too narrow. It is corrected by sufficiently dissecting the space where the previous implant was located and placing a graft.

# When the nasal bridge appears crooked

This occurs when the silicone placed on the nasal bones does not match the underside or shape of the nose, causing it to shift to one side. It is corrected by carefully shaping the portion that contacts the nasal bones to prevent shifting to one side.

# When the implant is visible through the skin

When an implant used in previous surgery shows through the nasal bridge or tip, the procedure involves replacing it with a new, thinner implant or wrapping the implant with fascia or artificial dermis before reinserting it.

# When the implant is thought to be moving

Even if the implant is not actually moving, if it remains a persistent concern, it can be corrected by placing the implant beneath the periosteum to match the contours and shape of the nasal bridge or by changing the implant.

# When the nasal bridge feels too wide or too narrow

Timing for Revision Rhinoplasty

Revision rhinoplasty is considered a highly challenging procedure because it must be performed when the tissues have already been damaged. Therefore, if planning revision surgery, it is advisable to wait until the skin tissues have softened sufficiently.

# General Timing for Revision Rhinoplasty

To minimize complications, it is advisable to undergo revision surgery after at least 6 months to 1 year, once the nasal tissues have stabilized.

# In Cases of Inflammation

If inflammation occurs, it is advisable to undergo revision surgery after the inflammation has subsided through injections, antibiotics, or other treatment.

# If the Implant Has Been Removed

If the implant has been removed, revision surgery involving implant insertion is performed after the wound at the removal site has fully healed.
